FORMS - PLEASE READ CAREFULLY
You must first become a member of the OHBS before you can take your horse to an inspection for breeding approval or registration. Please click on "Membership" to find the PDF form. Please send that in along with your membership fee payment to the OHBS office immediately. After you do this please sign up for the inspection and request the necessary forms by e-mailing one of the following appropriate e-mail addresses: Stallions@oldenburghorse.com , Mares@oldenburghorse.com, or Foals@oldenburghorse.com
 
Required Inspection Forms and Documents that must be fully filled out BEFORE you arrive at the inspection site:
 
Enrollment Form: This form is available as a PDF form on the link below, or you can request that it be sent to you by mail from the OHBS office.
 
The Foal, Mare, and Stallion forms: Please see the important notice about these forms below! You can either call having the following information available or e-mail this information to geri@oldenburghorse.com :
 
Mares: Mare's name and registration number (from the original breed registry), Date of birth, color, height, Dam's name and registration number, Sire's name and registration number, Breeder's name, Owner's name, Owner's mailing address, Owner's home telephone number, cell number, business number, fax number, and e-mail address
 
Foals: Foal's name following the naming rules, sex of foal, date of birth, date of conception (last breeding date of dam - extremely important) Sire's name and registration number, Dam's name and registration number, Breeder's name, Owner's name, Owner's mailing address, Owner's home telephone number, cell number, business number, fax number, and e-mail address
 
DNA Envelope: This is an envelope with a small form on the front of it that you will need to request to be sent to you by mail from the OHBS office. This must be filled out before you arrive at the inspection site. Samples for DNA can ONLY be pulled by the inspectors at the site, and they will seal the sample in the envelope at that time. Every horse being inspected for either registration or breeding approval with the Oldenburg Horse Breeders' Society must be, or have been DNA tested by the OHBS or the German Oldenburg Verband.

Required Documents: For stallion or mare breeding approval you must bring the ORIGINAL registration papers of the horse being inspected and a copy. For foal registration we need the completed ORIGINAL BREEDING CERTIFICATE as well as the dam's ORIGINAL papers. The breeding certificate can be obtained from the sire's owner or the frozen semen agent, and it must be complete with the date of shipping, date of insemination, and the inseminating vet or technician's name.

Inspection Site Forms/Documents: The host farm may require additional information for their records and may provide a secondary entry form to be returned prior to your inspection date. Contact your local inspection site to receive a copy of the form(s) for early completion and for stabling information. In order to prevent the passage of diseases please ensure that all breeding stock over 1 year of age have documented proof of negative coggins tests at the inspection and their vaccinations are up to date.
